What to do?

Just fill out your issue. It’ll take some time for Instagram to review the process. If the information matches their database, they’ll send you password reset link.
Fill in the details and click the Send for Instagram to review your case. It might take around 5 to 20 days for Instagram to review it and take action on it. If your appeal is not accepted, there is nothing much you can do apart from downloading your data. In this case, your disabled account will then be deleted. Just create a new one.
